Cardiac Rehab- Registered Nurse Full Time
Salem Medical Center is seeking a full time Cardiac Rehab Nurse to join the team! The duties of the Cardiac Rehab Nurse include:
-Reviews patient medical record.
Documents information on patient outcome form.
-Conducts admission interview with patient, spouse and/or significant other.
Establishes patient's physical and psychological needs to achieve desired outcomes.
Documents symptoms, medications, physical limitation, cognitive limits and learning needs.
-Establishes risk factor summary.
Completes nursing diagnosis (PPL) with plan for improved outcome.
-Prepares exercise prescription based on stress testing, physical limitations and degree of heart disease.
-Conducts equipment orientation with patients and performs physical assessment.
-Conducts exercise sessions and assesses response to exercise in order to adjust work loads appropriately.
-Collaborates with physician regarding referrals, prescriptions, home exercise, testing, dietary interventions and acts as a patient liaison when appropriate.
-Patient teaching is performed through hand-outs and verbal reinforcement, family participation in patients care plan is encouraged.
Provides reinforcement, family participation in patients care plan is encouraged.
Provides reinforcement for established problems by ongoing teaching and review of food diaries and updates of outcome sheet when needed.
-Completes discharge teaching and home exercise plan, documents and forwards a copy to the physician.
-Completes nursing notes following each session.
-Interfaces with other nurse resources in and outside the hospital which can affect an improved outcome.
-Plans and participates in community and rehab activities to educate in prevention and reinforce individual efforts.
-Performs all other duties and responsibilities as necessary.
Education:
Bachelor's or Associates level degree from an accredited school of nursing.
Experience:
Background or education in exercise in physiology preferred.
Licenses/Certificates:
Must be a Registered Nurse with a current NJ license.
AHA BLS or equivalent required.
ACLS certification required within 6 months of hire.
Arrhythmia and ECG interpretation are required.
Recommended Skills Advanced Cardiovascular Life Support (Acls) Basic Life Support Certified Nurse Practitioner Electrocardiography Medical Records Nursing Estimated Salary: $20 to $28 per hour based on qualifications.
